Here are the instructions for completing and presenting the Application for Registration and ID Card (Yellow Card) Form:
Once you have completed the form online using your PC, you can save it as a PDF file and then print it.
With the printed form:
1. Sign the form in the signature box,
2. Insert your thumbmark in the box where indicated,
3. Bring two (2) photographs for your Yellow card aplication.
You must submit the form personally at the Embassy but NO APPOINTMENT is required and you will be attended on a “first come-first served” basis.
